Step 6 - Health History / Immunizations

You will be asked to complete a health history form during orientation to the agency. Or you may print the form here. Some of our facilities require a physician statement of health so it is advisable to get the form signed by a physician, physician's assistant, or advanced practice nurse before submission.

The following immunization records are required:

  • Negative TB test within last year. AMP offers the PPD test to new and current employees, however without previous records two PPD tests are required.
  • Rubella titer or immunization
  • Rubeola titer or immunization
  • Mumps titer or immunization (required for some facilities)
  • Varicella titer, immunization or history
  • Hepatitis B immunization record or titer. If record is not available, you may sign a record form or a declination.

Immunization records may be available to you from your current or previous employer, education, OB, military or baby records.
